In order for a participant to be checked in, they must have completed the following:

Prior to GP

• must have paid registration fees for Goodphil

• must be on the roster provided by the FSA

  • If alumni, have their proof of recent graduation included (picture of diploma)

  • If currently enrolled, have an image of their class schedule

• have signed our online GP'22 COVID-19 SAFETY & INJURY RISK ACKNOWLEDGEMENT form.

• have printed out and signed the UH Rec’s waiver if an athlete (will be released soon)

At GP Check-in:

• must provide their government issued ID. 

• must provide a negative Covid-19 test 

• sign Covid-19 risk acknowledgement if not completed before GP

• must provide a printed and signed UH Rec waiver (will be released soon)

Each individual participant is responsible for providing a negative Covid-19 test and filling out the waiver.

A roster, however, must be provided by each FSA. For the roster, we request that each FSA use this template. Make a copy of the template and title it "[Insert School Here] 2022 Roster Sheet". On the roster sheet, you will see several columns of information that we request. Information regarding eligibility will be verified by SIFA.

To ensure a speedy check-in where participants only need to show their ID and covid test results, please have this information provided by each school by the Feb.25 deadline.
