Participant Check In List

CHECK IN FOR ALL PERFORMERS AND ATHLETES

Prior to GP

• have paid registration fees for Goodphil

• be on the roster provided by the FSA

• If alumni, provide proof of recent graduation included (picture of diploma)

• If currently enrolled, provide an image of class schedule

  • Most recent class schedule if a gap year student

• signed our online Covid-19 waiver form.

• printed out and sign the UH Rec’s waiver if attending sports days (athletes & non-athletes)

At GP Check-in:

• provide their government issued ID (Not student IDs). 

• Show a verifiable negative covid test

• provide a printed and signed UH Rec waiver if attending sports days

Each individual participant is responsible for providing a negative Covid-19 test and filling out the waiver.
